Greetings:

I write to inquire about the ability to create a data input page. I am trying to create a database for patient laboratory results. Obviously, the quality of data and so I would ask if there is a way for information contributors to enter their name for their entries, constraint lab results to allowable values, automate a timestamp for each entry, some form of encryption if the data is uploaded to a website. Is there a way to generate a unique identifer for all new patients. To summarize, I would like to customize an interface page for such data and export this custom page to other computers.
