Create PDF Form with user input text that then repeats

Hi, I've spent a couple hours looking through various forum threads, but I can't seem to get this to work, could someone help me?

I'm trying to create a PDF form that works in the following way:
1. User opens the PDF in Adobe reader
2. User sees an input field at the top of the page, where they input a number
3. At multiple places in the document, the number that the user entered is repeated.

So far, I've found that I should be using form fields? Like user or input fields? I can see how to create a field, which then gets repeated in the document. But I can't see how to create a field that allows a user to input text into, and then the text gets repeated... All of this should happen once the document has been converted to PDF

Still trying to fiddle around with this but can't figure it out... any open office gurus out there who know how to do this?
