Just learning, but could use some help

Hello,

So I'm building a database for my work, a little brown nosing, but I have never done anything like this before. I'm trying to make a daily production report form for employees. I have figured out a few things but could really use help. I uploaded a screen shot of the form with a description of the help I need. Any help would be GREAT. Thanks
