from document into form

Hi all, I spent a whole bunch of time making a document that looks exactly how I would like it to look. Used a bunch of tables, adjusted widths and got just the spacing I want for my form. I would like to now save it in such a way that I can type into the blank spaces, but I cannot mess up the layout by pushing return, etc.

I think I would call this converting it into a form, but not sure.

Is there a way that I can now save the document as a form?

You can save as a template (.ott) then you can mess up the file created from it but you won't change the template.
