Captain's Log background template anybd?

Hello,

I'm very pleased with OOos suite. Currently I'm using writer to write patrol reports for a forum I'm part of. I've set up the font that I want and I need a background of a notebook or a wide binder with lines and margins corresponding to writing lines, so it looks like a diary or a captains log.

In case you are not familiar, this is what I'm looking for as background to typed text,





and for finishing the works, I need to export the written text to an image format I could upload to my log forum.

Any ideas on how to do these? Thanks in advance
