I'm trying to do a report at my work and, when I edit this , at first page it's OK ! Title, image and things that I wrote, but from second page to the end, my name and the date were displayed ! How could I do to repeat the same header of the first page or, that the header was blank ?

The fields "Title", "Author" and "Date" appearing at the top of your second page and the following ones belong to the Page Header.
So, you can either de-activate this header (and have only a header in your first page) or add the first page header to all the pages.

1. Remove the second an following pages header :
the page style of the second page and following is "Standard". you will have to change it using the "style and formatting " button (or F11 key ), click on the "Page Styles" button, right click on "Standard" , choose "edit" then "Header" and you wil find a checkbox to desactivate the header

2. add the first page header to all pages
Same process but you need to edit the "First Page" style: in the first sheet of the window, under the "Name" Field (containing the name "First Page") , you can choose the following style : choose again "First Page"
