Repeating Logo and Header (was Master page)

Hi all,
On the 8th Jan, I asked about how to set up repeating headers. I have tried the
suggestions given on Templates and Styles, but can't seem to get them to work. I need to be able to put a Logo and Header on Page 1 of a document, and have them repeat automatically thereafter for as many pages as I like. How do I do this please?

I have posted in this forum sometimes ago regarding the same. However there is one caveat: the post is relation to another template for my old company which I do not have a copy with me now. Nevertheless the steps should be clear enough. Please do not hesitate to email or post reply here if you need further clarification.
