I just installed Base and I'm trying to move off MS Access to Open Office
However, the create form icon is greyed out? any thoughts as to why this is.
I have only installed the Data base aspect of the product - could this be the issues? |

Yes,
You can not install only Base, you must at a minimum also install the Writer module.
