Hi,
I'm looking to create inspection forms with Open Office and I'm having trouble adding checkboxes to the document body.
For example:
The inspection form is designed to ask a question, to which the answer is either "Yes", "No", or "N/A". Each possible answer will have it's own checkbox which an inspection technician will select based on the condition of the system being inspected.

Select from the menu View → Toolbars → Form Controls.
It should now appear with a tick beside the name when you look at the menu.
You should also see the toolbar in your OpenOffice.org Writer window. if you have trouble finding it, click again on View → Toolbars → Form Controls to make it disappear, and again to make it reappear. You should eventually notice what is appearing and disappearing.
The second icon on the toolbar, persuming you haven’t changed it in any way, is the Activate icon. If it its not already activated, click on it. When activated the icon will be surrounded by a rectangle and appear darker.
Clicik on the checkbox icon to activate it. Then use your mouse pointer to draw a horizontal rectangle approximately where you want the check box to appear. You should now see the check box, with green handles. Click on it, and select from the context menu “Control...”. In the Control window you can edit the Label field to set the check box text to whatever you wish.
Other controls are available. See “Check box” creation under “Index” under the menu item Help → OpenOffice.org Help for further details.
When you have the check boxes set as required, you can click again on the “Activate”' icon on the Form Controls toolbar to turn the screen back to normal mode, in which the check boxes will work as you expect. When fully satisfied, click on the X in the upper corner of the Form Controls toolbar to delete it (at least for the moment). |

Hey thanks for the quick response.
Will the checkboxes show up if I print the page, or is it designed for an html application?
Next question, can I add a check to the checkbox in Writer? The idea being that blank question boxes would be taken to the jobsite and checked off by hand by the inspection technician, and then at the office, the reports would be reproduced, printed off, and sent to the client with all appropriate boxes checked off via Writer.
